Work Permits

1) AFTER you have been offered a job, please fill out the following form: Student work permit application for 14-17 year olds 

Please note that the employment permit application must be signed by the employer, parent, and student.  

If the student attends DSHS or DSMS we do not need a physician's signature as we have this on file in the nurse's office.  Non-Dover-Sherborn students should submit a copy of your most recent physical.  

2) Please bring your completed applications with the signatures listed above, to the Dover-Sherborn Central Office (157 Farm Street, Dover; the gray building in front of the middle school).  The office is open during the school year from 8am-4pm and during the summer, 8am-3:30pm Monday-Thursday and 8am-12pm on Fridays.  The student will need to be present in order to sign the work permit.
