Work Permits

1) AFTER you have been offered a job, please fill out the following form: Student work permit application for 14-17 year olds 

Please note that the employment permit application must be signed by the employer, parent, and student.   

2) Please bring your completed applications with the signatures listed above, to the Dover-Sherborn Central Office (157 Farm Street, Dover; the gray building in front of the middle school).  The office is open during the school year from 8am-4pm and during the summer, 8am-3:30pm Monday-Thursday and 8am-12pm on Fridays.  The student will need to be present in order to sign the work permit.
