Immunization Requirements

Physical Examinations: The Dover-Sherborn Public School Health Program ensures that every student be separately and carefully examined by a duly licensed physician, nurse practitioner or physician assistant, as follows:

  • within one year prior to entrance to school or within 30 days after school entry and at intervals of either three or four years thereafter. 
  • The Dover-Sherborn School Health Program has designated that these intervals be prior to kindergarten, third grade, seventh grade, and tenth grade. A student transferred from another school system shall be examined as an entering student. Health records transferred from the student's previous school may be used to determine compliance with this requirement

Screenings: The following screenings will be performed according to the Massachusetts Department of Health:

Students with Special Medical Conditions:

Please contact the school nurse as soon as possible if your child has a special medical condition. A special medical condition may include the following: asthma, diabetes, seizures or a life-threatening allergy. In order to create a safe environment for your child, the nurse will meet with you and make an Individual Health Plan (IHP) for your child if needed. This plan will include input from the parent, the nurse and the child's health care provider.
