Physical Exams & Immunization Records
All children entering kindergarten and grade three and all other new students must provide a record of a physical examination done since the previous September. This is due in the Health Office before the opening of school. In addition, all kindergartners and other new students must provide a complete immunization record signed by a licensed health care provider. |
